[TYPES] Wanted: Nancy McCracken's Ph.D Thesis - An Investigation of a Programming Language with a Polymorphic Type Structure

I recently got her Ph.D Thesis scanned from Microfiche by ProQuest.
But unfortunately it is missing a page, page 113.

This is a seminal work as she introduces the notion of Kinds.

I am wondering if anyone has a complete copy in their archives please ?
